Job Description
Social Worker (Community Care Flex)

Community Care Flex Social Worker will provide social work coverage when needed, i.e. extended absences, vacancies, etc. to any of the following programs: Contract Nursing Home (CNH), homemaker/Home Health Aide (H/HHA), Contract Adult Day Health Care (CADHC) and In-Home Respite Program. Individuals assigned to the GS-12 level must be licensed at the advanced practice level and have a Masters Degree in Social Work that is fully accredited by the Council on Social Work Education (CSWE).

The VA Northern California Health Care System (NCHCS) Community Care Programs contracts with home health agencies, nursing homes and Adult Day Health Cares to provide paid attendant care to eligible veterans living at home, placement. This service is provided in counties in the NCHCS catchment area. The Coordinator is an advance practice social work coordinator who is primarily responsible for the management, integration and evaluation of these programs at the NCHCS. In addition to these duties, Coordinator has clinical responsibilities and provides social work services to eligible veterans, either in the context of program management, or as an adjunct social worker in
the Community Care Program.

The Flex social work/ coordinator evaluates Community Care consults and assess the veteran's eligibility for services based on established criteria, including the Respite Handbook and the Purchased Home Health Care Services Handbook as well as NCHCS policies pertaining to these programs. The social work coordinator assesses the appropriate services for the veteran based on his/her needs, the recommendation(s) of his/her VA providers, and the availability of contracted services. The social work coordinator reviews the plan of service as outlined for the program being covered. Ranges can be: every 6 months, monthly or more often as needed, and is in regular contract with the providing community partner and the veterans in the program(s) to address problems, or issues with the goal that the service helps support the veteran in his/her home setting.

The social work coordinator is aware of federal, state and JCAHO regulations pertaining to home health agencies, and ensures that agencies are in compliance with all pertinent regulations. The social work coordinator audits agency records as needed to ensure compliance with these regulations.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English language proficiency: Must be proficient in spoken and written English
  • Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if degree meets accreditation standards for a master of social work. (COPIES OF TRANSCRIPTS MUST BE SUBMITTED WITH APPLICATION)
  • Licensure: Must have a current and active license or certification by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless grandfathered by the state which you are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). (COPIES OF LICENSE AND CERTIFICATION MUST BE SUBMITTED WITH APPLICATION)

Grade Determinations:
SPECIALIZED EXPERIENCE FOR GS-12 LEVEL: Individuals assigned as a senior social worker must have experience that demonstrated possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice. I have a minimum of at least two years of specialized experience demonstrating this type of experience.
