Job Description
Clinical Pharmacist

The Clinical Pharmacist (CP) in this assignment manages routine medication-related activities in accordance with VHA (Veteran Health Administration), DEA, FDA, Joint Commission, Veteran Integrated Service Network (VISN) and VA Maryland Health Care System (VAMHCS) and other governing regulations, standards and policies. The incumbent has key responsibilities in pharmacy distribution and operations, formulary management and medication safety. As such the CP is responsible for reviewing, interpreting, and verifying medication orders for appropriateness; along with processing and dispensing of inpatient and outpatient medications orders. The CP interacts with other clinical staff, making recommendations regarding medication therapy ordered to ensure safe and effective care. The CP provides oversight of technical staff in all aspects of medication distribution. The CP will represent Pharmacy Service on VAMHCS and VISN-level committees as assigned, participates on interdisciplinary team meetings/clinical rounds, communicates recommendations to the team when appropriate, and will serve as preceptor to students of affiliated schools, VA Learning Opportunity Residency (VALOR) students and Pharmacy Practice residents. The CP may be assigned to various pharmacy sections/divisions within VAMHCS Pharmacy Service, as determined by scheduling and workload requirements in order to provide pharmacist coverage on all operating shifts. Incumbent functions as a member of a team to provide optimal pharmaceutical care and timely customer service in accordance with the mission, vision and goal of the VHA, VISN and VAMHCS.


Work Schedule: Sunday - Saturday with varied hours (up to 7 days per week and tour of duty varies on the needs of the service. We offer 24 hour, 7 days per week service, so some overnight shifts may be required). Initially, after training is completed the assigned tour will be 7:00 am to 3:30 pm Monday through Friday with weekend and holiday rotation.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
Grade Determinations:
GS-12 Clinical Pharmacist (Full Performance Level)

(a) Experience or Education. In addition to the basic requirements, candidates must meet one of the following:

1. 1 year of experience equivalent to the next lower grade level, or

2. Completion of an ACPE-accredited Pharm.D. program.

(b) Assignment. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):

1. Knowledge of professional pharmacy practice.

2. Ability to communicate orally and in writing to both patients and health care staff.

3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.

4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

The full performance level of this vacancy is GS-12.Physical Requirements: A pre-placement physical examination is required for all full-time, part-time and intermittent employees identified by VA Directive and Handbook 5019. The pre-placement physical examination shall be completed prior to appointment to determine the physical and mental fitness of candidates for appointment in VA. The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies.
