Job Description
Mid-Level Provider (Orthopedics)

Routine Clinical Duties:

Perform and document history and physical examinations.

Document data from outpatient and or inpatient visits

Writing initial and subsequent outpatient and inpatient orders and progress notes including consultations to subspecialties.

Order diagnostic tests.

Determines initial medical assessment and develops treatment plans according to accepted protocols and physician supervisor's recommendations.

Prescribes medications according to approved formulary.

Participates in clinic and writes clinic notes.

Educates patients concerning medical conditions and recommended treatments. Specific topics include nutrition counseling, need for smoking and alcohol cessation, need for periodic immunizations, and health screening tests.

Participates in the Operating Room in the role of Surgical First Assistant as outlined per their O.R. Privileges

Non-routine non-emergent Duties:

Perform approved procedures when indicated and requested by PA and approved by clinical supervisor.

Procedures are outlined per privileges.

Provide medical care to patients assigned to all Surgical Service clinics when needed due to staff shortages.

Perform monthly chart reviews as assigned.

Emergency Duties:

Such duties necessary to maintain the life of patients under the PA's care until relieved of these duties by the physician supervisor. These procedures can include resuscitative measures such as basic Life Support and Advanced cardiac life Support (as certified).

Administrative Duties:

Serves as physician extender in specialty surgical service clinics. He/she evaluates patients in an outpatient and inpatient setting through assignments made by the Chief, Surgical Service or supervising physician. He/she sees patients requiring history and physical exams, urgent evaluation, pre-operative clinics, and assists in follow up clinics. PA serves as a point of contact for evaluation and/or referral.

Serves on medical center committees as assigned.

Serves as medical consultant to surgical service staff members who need to confer on assigned patients (Social Work, Nutrition and Food Service, Pharmacy, etc.).

Interacts with patients and medical staff when there are problems between patients and members of our staff that interferes with the efficient operation of our clinics.

Assists in the orientation of new employees who are assigned to the Orthopedic Surgery Section.

Work Schedule: Generally, Monday through Friday, 8:00 am - 4:30 pm.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a PA training program, or of a surgical assistant program which is accredited by the
    Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor organizations; AND have a Bachelor's degree from an accredited PA training program or in another health sciences related field from an accredited college or university recognized by the U.S.
    Department of Education;
    OR
  • Graduate of a school of professional nursing approved by the appropriate State agency and accredited by
    NLNAC or CCNE at the time the program was completed.; AND a Master's Degree from a program accredited by The National League for Nursing Accrediting Commission (NLNAC; http://www.nlnac.org ) or The Commission on Collegiate Nursing Education (CCNE; http://www.aacn.nche.edu)
Current, full, active and unrestricted certification as a physician assistant by the National Commission
on Certification of Physician's Assistants (NCCPA);
OR
Current, full, active and unrestricted license as a professional nurse in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States and appropriate licensure as a Nurse Practitioner and possess current, full, active and unrestricted certification as a nurse practitioner from the American Nurses Association (ANA) or another nationally recognized certifying body.

Grade Determinations
NURSE PRACTITIONER:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.

Experience will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

The criteria are as follows:
  • Practice: practice, ethics and resource utilization;
  • Professional Development: performance, education/career development;
  • Collaboration; collaboration, collegiality;
  • Scientific Inquiry: quality of care, research.
PHYSICIAN ASSISTANT

FULL GRADE - None beyond the basic requirements.


INTERMEDIATE GRADE -
Experience/Education

(a) 1 year of professional practice as a PA; or
(b) Successful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces; or
(c) A Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education.

SENIOR GRADE -
Experience / Education


(a) 3 years of professional practice as a PA; or
(b) 2 years of professional practice as a PA and successful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces; or
(c) 2 years of professional practice as a PA and a Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education; or
(d) A Doctoral degree in a health-related field from an accredited college or university recognized by the U. S. Department of Education.

CHIEF GRADE
Experience/Education


(a) Generally, an individual in this grade is able to demonstrate the behaviors listed in the areas below after 5 years of professional practice as a PA; and,
(b) The minimum education for this grade is a Master's Degree from a PA training program accredited by ARC-PA or in a health-related field of study. However, experience as a PA may be substituted for the Master's degree where the individual has an equivalent knowledge of the profession and has successfully undertaken a combination of difficult or complex assignments in several clinical, administrative, research and educational arenas which required a high degree of competence.

Preferred Experience: Demonstrated ability to work with Orthopedic surgeons and residents and other midlevel providers and a strong commitment to the vision of the VA - Care of the Veteran, teaching and research; and the ability to work with Providers from all other specialties.

The full performance level of this vacancy is RN Grade 3 or Chief Grade 13 The actual grade at which an applicant may be selected for this vacancy is in the range of Grade 1 to 3 for RN and Grade 10 to Grade 13 for PA.Physical Requirements: In the process of providing patient care, the work involves regular and recurring periods of walking, standing, sitting, bending, pulling, lifting, reaching, and carrying. Manual dexterity is required to operate medical equipment and perform activities such as treatments and computerized medical record documentation. Depth perception; Ability to distinguish basic colors; Ability to distinguish shades of colors; Hearing (Aid permitted); Near vision correctable at 13" to 16"; and Working closely with others.

Influenza Vaccination: Participation in the seasonal influenza program is a condition of employment and a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). HCP are individuals who, during the influenza season, work in VHA locations where patients receive care or who come into contact with VA patients as part of their duties.
