Job Description
Registered Nurse (Consultation Care Service-Transplant Coordinator)

The West Texas VA Health Care System is currently accepting applications for the position of Registered Nurse (RN) Transplant Coordinator.The RN Transplant Coordinator is the facility point of contact for interfacility transfer, solid organ and bone marrow transplants and travelling Veterans. Coordinates interfacility transfers for Veterans from the West Texas VA or its Community Based Outpatient Clinics. Provides case management and coordination of work up for transplant for Veterans in need of solid organ or bone marrow transplant. Provides ongoing case management services for Veterans post-transplant. Coordinates with other VHA travelling Veteran coordinators to provide services for Veterans who are travelling to the West Texas area or for WTVAHCS Veterans who travel to other VA locations. Maintains case management documents of care coordinated. Provides back up case management with nurses following Veteran care in community facilities within the WTVAHCS catchment area.

Work schedule:8:00 a.m. - 4:30 p.m. Monday through Friday and may vary depending on the hiring facility.

BASIC REQUIREMENTS:

-U.S. citizenship
-English language proficiency
-Registration - Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full active and unrestricted registration to continue employment with the VA.
-Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed: a) The National League for Nursing Accrediting Commission (NLNAC); or b) The Commission on Collegiate Nursing Education (CCNE).
GRADE DETERMINATIONS: Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

The criteria are as follows:

Practice: Practice, ethics and resource utilization;

Professional Development: Performance, education/career development;

Collaboration: Collaboration, collegiality; scientific inquiry; quality of care, research.

Nurse I, Level 1 - Associate Degree or Diploma in Nursing with no experience.

Nurse I, Level II - ADN or Diploma in Nursing or ADN or Diploma in Nursing and Bachelors degree in a related field or Bachelor of Science in Nursing (BSN).

Nurse I, Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related filed and approximately 2-3 years of experience; or a BSN with approximately 2-3 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.

Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 2-3 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.

Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

Preferred Qualifications:
-At least5 year of basic Nursing practice and a BSN.

-Broadly acquainted with clinical treatment modalities.

-Have a basic understanding of the following:

a)The organization and administration of multifaceted health care delivery systems

b)Specialty and subspecialty care

c)Educational principles

d)Database management

e)Accreditation standards

f)Utilization review and/or management
SALARY: Applicant's education and experience will be reviewed by a Professional Standards Board in determining the grade and salary of the applicant selected. The recommended salary may be at any point in the range listed for this vacancy.
PHYSICAL REQUIREMENTS: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling and pushing.
