Job Description
Registered Nurse - ICU

The incumbent practices as a registered nurse in the ICU/Step-down Unit, Nursing Section of Patient Care Services within the Detroit VAMC. The ICU/SDU is a specialty care unit that's scope of care includes postoperative surgical and medical patients requiring advanced care services. Their primary diagnoses include cancer, peripheral vascular disease, urologic disorders, respiratory failure as well as cardiac disease and other medical illnesses. The nurse demonstrates knowledge and competency in critical care nursing skills such as hemodynamic monitoring, early recognition of cardiac lethal dysrhythmias and effective treatment of life threatening cardiac complications.

*The nurse follows approved ICU/SDU protocols and works collaboratively with all members of the interdisciplinary ICU/SDU team.

*Must be comfortable covering ICU and Step-Down patients if the need arises.

*The incumbent must assess and monitor the patient closely in order to identify subtle changes in a patient's condition that warrant immediate intervention.

*Patients admitted tend to be medically unstable, requiring constant cardiac and respiratory monitoring and continual adjustment of treatments, such as the titration and dosing of multiple intravenous medications and changes in ventilatorary support.

*The RN must be able to interpret, integrate and respond to a wide array of clinical information. Because of the critical nature of patients' conditions, nurses working in these areas are often confronted with dealing with end-of-life issues and sometimes other ethical dilemmas related to withholding, withdrawing or medical futile care.

*Functions as a consultant and mentor to other nurses.

*Educates colleagues and/or students and serves as a preceptor.

*Participates as a member or assumes a leadership role on formal service level committees, councils, or performance improvement teams.

*The RN is responsible for, but not limited to, the following major responsibilities:

*Chest tube procedures/care *Pacemaker

*Moderate Sedation *Intubation and ventilator management

*Central/PICC Line care and dressing changes *Intra-abdominal Pressure monitoring

*Right Atrial Pressure Monitoring *RME

*Blood Transfusions *TPN

*PA catheter and pressure line troubleshooting *Pacemaker Permanent/Transvenous

*Stroke protocol *Vasoactive drug monitoring and titration

*Epidural catheter insertion, management and removal

*Swanz Ganz and Arterial Pressure based Cardiac Output

Work Schedule: Possible tours of duty: 7:00a-7:30p, 11:00a-11:30p, 7:00p-7:30a rotating days to include weekends and holidays.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
    • The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: ICU/Step-Down Unit Experience

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.

This can be found in the local Human Resources Office.Physical Requirements: Heavy Lifting, 45 pounds and over, Heavy carrying, 45 pounds and over, Straight pulling (4 hours), Pushing (4 hours), Reaching above shoulder, Both hands required, Walking (8-12 hours), Standing (8-12 hours), Repeated bending (8 hours), Both legs required, Ability for rapid mental and muscular coordination simultaneously, Near vision correctable at 13" to 16" to Jaeger 'I to 4, Far vision correctable in one eye to 20/20 and to 20/40 in the other, Both eyes required, Ability to distinguish shades of colors, Hearing (aid permitted), Transferring patients & objects may be required, Don protective clothing in isolation situation or operative/ invasive procedures, May occasionally be exposed to patients who are combative secondary to delirium.
