Job Description
Physician Assistant - Home Based Primary Care

The Iowa City VA is currently recruiting for aPhysician Assistant for the Home Based Primary Care Program. This position will be under the supervision of the HBPC physician; the PA:

  • Functions as a care manager with primary medical responsibility for their assigned patients.
  • Delivers care in the home.
  • Assesses care needs of patients and caregivers.
  • Identifies and defines the medical management of patients' medical problems.

  • Determines the need for consultation from subspecialty clinics.

  • Determines the need for, and facilitates admission to the hospital.

Work Schedule: This position will primarily be a day shift, Monday through Friday, 8 hour shifts, between the hours of 8:00AM to 4:30PM, with intermittent, unplanned extensions of the regular tour of duty as patient care needs dictate.

Functional Statement Title: HBPC Physician Assistant

Basic Requirements:

Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)

Education or Training. The candidate must meet one of the following:

(1) A bachelor's degree from a PA training program which is certified by the Accreditation Review Commission on Education of the Physician Assistant (ARC-PA); or

(2) Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a bachelor's degree in a health care occupation or a health-related science; or

(3) Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a period of progressively responsible health care experience such as an independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist, or medical technician. The duration of approved academic training and health care experience must total at least 5 years.

Certification. As of March 12, 1993, certification by the National Commission on Certification of Physician's Assistants (NCCPA) is a requirement for employment. This requirement does not apply to non-certified PAs on VA rolls as of that date.

Physical Standards. Applicant must meet the physical standards established for this position.

English Language Proficiency. PAs appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Preferred Experience:

  • 2 to 3 years of experience as a Physician Assistant.
  • Prior home care experience

Grade Determinations:

Full Grade (GS-9)- No requirements needed beyond the basic requirements listed above.

Intermediate Grade (GS-11)- In addition to the basic requirements listed above, PA's appointed or advanced to this grade will be able to demonstrate the ability to analyze and solve more complex medical problems and have at least one year of professional practice as a PA. -OR- Have successfully completed a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces. -OR- Have a Masters' degree from a PA training program certified by ARC-PA or in another health-related field from an accredited College or University recognized by the U.S. Department of Education.

Senior Grade (GS-12) - In addition to the basic requirements listed above, candidates must meet all requirements for the intermediate grade, PA's appointed or advanced to this grade will have demonstrated the behaviors or responsibilities listed in one or more of the following categories: Clinical Proficiency, Education, Professional Development, Management/Administrative , and Research (as applicable to the assignment). -AND- Have at least three years of professional practice as a PA. -OR- Have at least two years of professional practice as a PA and successful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces. -OR- Have at least two years of professional practice as a PA and a Masters' degree from a PA training program certified by ARC-PA or in another health-related field from an accredited College or University recognized by the U.S. Department of Education. -OR- Have a Doctoral degree in a health-related field from an accredited College or University recognized by the U.S. Department of Education.

Chief Grade (GS-13) - In addition to the basic requirements listed above, candidates must meet all requirements for the senior grade. Appointment or promotion to Chief Grade is based on the individual's demonstrated high level of clinical expertise, leadership ability, and the ability to function with a high degree of autonomy. Furthermore, the PA candidate for the Chief Grade will have demonstrated all the requirements of one or more of the following categories: Management/Administrative; Clinical Proficiency; Professional Development and Research. Generally, an individual in this grade is able to demonstrate the behaviors listed in these areas after five years of professional practice as a PA. -AND- The minimum education for this grade is a Masters' degree from a PA training program accredited by ARC-PA or in a health-related field of study. However, experience as a PA may be substituted for the Masters' degree where the individual has an equivalent knowledge of the profession and has successfully undertaken a combination of difficult or complex assignments in several clinical, administrative, research and educational arenas which required a high degree of competence.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Physical Requirements:It is VA policy to ensure that all selected applicants and employed personnel are physically, mentally and emotionally fit to perform the duties of the position to which assigned. A pre-placement physical examination is required for all full-time, part-time and intermittent employees identified by VA Directive and Handbook 5019. The pre-placement physical examination shall be completed prior to appointment to determine the physical and mental fitness for candidates for appointment in VA. The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies. The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. ICVAHCS is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Qualifications & Requirements
