Job Description
Licensed Practical Nurse

The Primary Care Licensed Practical Nurse provides direct staff support to the Primary Care Team Physician and the Primary Care Medical Clerk in planning, designing, integrating, implementing, modifying, and administering the Primary Care Model of Health Care at the Medical Center and is a key leader in the Primary Care Team.

Duties include but are not limited to the following:

  • Identifies, initiates, and actively participates in safety and emergency measures related to patient internal and external issues.
  • Maintains confidentiality in the delivery of all aspects of care.
  • Maintains a safe environment while employing safe practices (e.g., body mechanics, PPE) compliant with all rules and regulations as defined and mandated by the safety policies of the Medical Center.
  • Documents observations and information related to patient care via computer per policy and procedure.
  • Initiates patient appointments through hospital computer programs (VISTA or CPRS) by use of the scheduling menus to schedule lab, x-ray, and EKG tests with other clinical services, necessary for physician clinics.
  • Explains necessary test preparation to patients and records information such as date, exam type, and procedure required in the DHCP and on appointment card or plastic ID card.
  • Schedules and coordinates return appointments for veterans, sending computer generated appointment letters or by personal telephone call.
  • Makes clinic corrections and changes as requested by patient and/or physician. Upon canceling clinics and appointments, reschedules these cancellations and makes notification of change through computer generated letters or by phone utilizing computer generated information, ensuring that all this information is accurately entered into the VISTA system.
  • Communicates and interacts in a professional manner with all internal and external customers.
  • Provides timely feedback in addressing issues while utilizing all services for satisfying goals.
  • Maintains confidentiality of electronic, written, and/or verbal patient/employee information in accordance with policies and procedures of the Medical Center.

Work Schedule: Monday through Friday, 7:00am - 3:30pm or 8:00am - 4:30pm

Basic Requirements:

Citizenship: Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Education: Graduate of a school of practical or vocational nursing approved by the appropriate state agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Additional information may also be obtained from the NLNAC Website. Exceptions include:

1. Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
2. The education requirement is waived for VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions.
3. Individuals granted a license by a jurisdiction that does not require graduation from an approved school.

Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.

English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Grade Determinations:

GS-6.

  • Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent , fully meeting all performance requirements for the GS-5 LPN/LVN.
  • Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.
  • Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize review by a registered nurse (RN) or physician (MD/DO) for appropriateness and conformity with established policies and procedures.
  • Ability to observe, identify and respond to the patient's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related] factors.
  • Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO.
  • Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.
  • Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.
  • Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery. Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers.
  • Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.
  • Actively seeks out education opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Physical Requirements: Functional requirements include the following abilities: Heavy lifting, 45 pounds and over; heavy carrying, 45 pounds and over; straight pulling one (1) hour; pushing; reaching above shoulder; use of fingers; both hands required; walking seven (7) hours; standing seven (7) hours; kneeling 1/2 hour; repeated bending; both legs required; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; ability to distinguish basic colors; ability to distinguish shades of colors; hearing without aid; mental/emotional stability; Environmental Factors required: Working outside and inside; dust; working with hands in water, working closely with others and protracted or irregular hours of work.

Participation in the seasonal influenza program is a condition of employment and a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). HCP are individuals who, during the influenza season, work in VHA locations where patients receive care or who come into contact with VA patients as part of their duties. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased facilities. HCP include all VA licensed and unlicensed, clinical and administrative, paid and unpaid, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, students, researchers, volunteers, and trainees who work at these facilities.
