Job Description
Registered Nurse (HUD-VASH)

The Louis Stokes Cleveland Veterans Affairs Medical Center is recruiting for a Registered Nurse. The Registered Nurse will function in the Comprehensive Homeless Center. The incumbent functions as a Housing and Urban Development-Veterans AffairsSupportive Housing (HUD-VASH). This position will be based in Canton, but will require extensive travel in a government vehicle coveringStarkCounty.The Registered Nurse functions as an intensive case manager for chronically homeless Veterans, a special population with highly complex or mental health problems. The Registered Nurse duties include:

  • Providing case management based services, serving as primary contact with local Public Housing Agencies (PHAs) to ensure Veteran care is coordinated with the VA Medical Center, community health or mental health providers and agencies that serve homeless populations.
  • Providing clinical consultation, nursing and clinical oversight and community collaboration for multiple community landlords who accept HUD low income vouchers.
  • Interviewing clients and conducting psychosocial assessments.
  • Interpreting and explaining VA treatment and benefit programs.
  • Assessing high risk factors, acuity and need for services.
  • Planning effective treatment in coordination with interdisciplinary team.
  • Implementing treatment plans and ensuring that Veteran receives needed individual or group counseling services.
  • Evaluating, educating, coaching and mentoring Veteran to ensure progress towards goals and adjusting the treatment plan as needed.
  • Documenting medical records including assessments, treatment plans, progress notes, follow ups and referrals.
  • Conducting and participating in outreachactivities.
  • Applying knowledge of housing quality standards for inspections to ensure Veterans reside in a safe environment.

Work Schedule: Monday - Friday, 8:00am - 4:30pm

In order to qualify for this position you must meet the following requirements:

  • Citizen of the United States.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by ofthe following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full and unrestricted registration will meet the requirements of graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in any State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
  • Must possess a valid state issued driver's license and have the ability to transport Veterans in a government vehicle.

Education/Experience Requirements:

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with additionalnursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience,OR an ADN or Diploma in Nursing and a bachelor's degree in a related fieldwithno additional nursing practice/experience;OR a Bachelors of Science in Nursing (BSN)withno additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience,OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience;OR a BSN with approximately 1-2 years of nursing practice/experience,OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience,OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experienceOR a Master's degree in Nursing or related field with a BSN and approximately 1-2 years ofnursing practice/experience,OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience;OR aDoctoral degree in Nursing or related field and approximately 2-3 years of nursing practice/experience.

Preferred Experience:

  • Experience working in the community.
  • Experience working with the homeless who may have mental health and substance dependence.
