Midlevel Provider- Comminuty Living Center

Assigned to Community Living Center under the Medical Service:

Physician Assistants (PA) and Nurse Practitioners (NP) are health care professionals who perform a broad spectrum of patient care services under the supervision of and/or collaboration with a physician. The nurse practitioner/physician assistant assesses, plans, implements, and evaluates the needs and care of older adults with acute and chronic diseases. In addition to providing direct resident care, the nurse practitioner also provides resident and family teaching, staff education, and consults and collaborates with other health care providers. He/she participates in nursing and medical research; prescribes medications and adjunctive modalities; and refers residents for specialty care when necessary. The Nurse practitioner/physician assistant serves as an expert clinician, executing position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. The Nurse practitioner/physician assistant demonstrates leadership in delivering holistic care through collaborative strategies. The Nurse practitioner/physician assistant manifests a high level of expertise in the assessment, diagnosis, and treatment of the residents' actual or potential health problems. Additional duties include:

a. Sought out for their expertise in the care of older adults with complex medical and psychiatric co-morbidities.

b.Maintains current knowledge of practice standards and guidelines related to adult and geriatric nursing, including the care of the cognitively impaired.

c.Develops plans of care by assessing the resident, establishing mutual goals with the resident and family, monitoring and evaluating each resident's progress toward care goals, and revising the plan of care based on ongoing evaluation.

d. Collaborates with the interdisciplinary treatment team in developing a resident centered plan of care that focuses on the resident's quality of life and care preferences.

e. Initiates contact with and collaborates with primary providers and other disciplines in order to coordinate each resident's care, including making appropriate referrals, communicating about the resident's status, identifying and removing barriers to care, exploring motivational and adherence issues when identifying these issues, and evaluating the resident's support systems.

Work Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m. Hours may be rotated or extended based upon the needs of the Medical Center or our Veterans.

BASIC REQUIREMENTS:

Citizenship - United States citizen.

English Language Proficiency - Proficient in spoken and written English.

Certification - Possess a current, full, active and unrestricted nursing license or certification by the National Commission on Certification of Physician's Assistants (NCCPA) is a requirement for employment. (This requirement does not apply to non-certified PAs on VA rolls as of that date).

EDUCATION/EXPERIENCE REQUIREMENTS:
Possess a MSN degree from a program accredited by the National League of Nursing Accrediting Commission (NLNAC) or the Commission on College Nursing Education (CCNE) or I hold a bachelor's degree from a PA training program which is certified by the Accreditation Review Commission on Education of the Physician Assistant (ARC-PA); or graduated from a PA training program of at least 12 months duration which is certified by the ARC-PA and a bachelor's degree in a health care occupation or a health-related science; or graduated from a PA training program of at least 12 months duration which is certified by the ARC-PA and a period of progressively responsible health care experience such as an independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist, or medical technician. The duration of approved academic training and health care experience must total at least 5 years.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Preferred Experience: Palliative Care, Geriatrics

Physical Requirements:

Physician Assistant: The work requires light lifting, under 15 pounds; light carrying, under 15 pounds; use of fingers; both hands required; walking; standing; repeated bending; Both legs required; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; ability to distinguish basic colors; hearing (aid permitted); mental and emotional stability.

Environmental Factors:

Working closely with others; protracted or irregular hours of work; protective clothing; gowns/gloves; exposure to infection and contagious diseases.
