Job Description
Registered Nurse - Care in the Community Clinical Coordinator

The Sioux Falls VA Health Care System is currently recruiting for a Full-time, Permanent, Registered Nurse - Care in the Community Clinical Coordinator. This position will be located in the Carein the Community (CITC)Service. As a Registered Nurse - CITCClinical Coordinator, you are responsible for the following duties to include, but not limited to:

  • Demonstrates leadership in assisting other staff to assess and evaluate patients' post-hospital nursing care needs.
  • Assists staff with planning by determining appropriate community resources.
  • Focuses on the patient in the context of family/community by integrating an assessment of living conditions, individual and family dynamics, and cultural background into the patient's plan of care.
  • Follows Delegation of Authority from Chief of Staff in approving and coordinating Non VA Community Care for Veterans.
  • Tracks care of Veterans by consult category for ongoing care management and discharge from care.
  • Facilitates scheduling of approved follow-up care by Care in the Community schedulers.
  • Complies with health, safety and environmental rules and procedures.
  • Performs work in a manner that will enhance the safety of the work environment.
  • Ensures strict adherence to federal, state and local codes concerning health, safety and the environment as applicable within the performance of his/her duties.

NOTE: Assignment to other areas of the service line, medical center, or other CBOC's may be required. TheRegistered Nursereports to theCITC Nurse Manager and works in collaboration with his/her assigned CORE team, however, assignments may be made to other areas for purposes of cross coverage.

Work Schedule: The tour of duty for this position typically is Monday - Friday, Days, Administrative Hours. Additional information regarding the tour of duty for this position will be discussed during the interview process.

Functional Statement: Registered Nurse

Basic Requirements:

  • Citizen of the United States.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
    - The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Must be proficient in spoken and written English.

Grade Determination: Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. The recommended salary may be at any point in the range listed for this vacancy.

Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

The criteria are as follows:

  • Practice: practice, ethics and resource utilization
  • Professional Development: performance, education/career development
  • Collaboration: collaboration, collegiality
  • Scientific Inquiry: quality of care, research.

Education/Experience Minimum Requirements:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.

Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

Preferred: Two years Med/Surg experience, Home Care experience, or Utilization Management experience.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

PHYSICAL REQUIREMENTS: The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. Sioux Falls VAHCS is an interdisciplinary teaching facility, offering medical, surgical , and mental health inpatient services. Comprehensive health care is provided through primary care, tertiary care, and long-term care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, geriatrics, and transitional care. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring Veterans and objects may be required. The RN may be exposed to infected Veterans and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The RN may occasionally be exposed to Veterans who are combative secondary to delirium, dementia, or psychiatric disorders. The RN must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on Veteran needs.
