RN - Staff Nurse, Home Base Primary Care (HBPC), Rehabilitation and Extended Care Line (Conroe, TX)

This Home Base Primary Care (HBPC) Registered Nurse (RN) isa part Rehabilitation and Extended Care Line withConroe as their duty station.

The HBPC RN makes decisions based on professional standards and scope of practice (i.e., Nurse Practice Act, ANA standards). This RN provides care and counseling of persons in the promotion and maintenance of health and illness/injury prevention based upon the nursing process. This includes systematic data gathering, assessment, appropriate nursing judgement, and evaluation of human responses to actual or potential health problems. This is accomplished through case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering treatments prescribed by a licensed or otherwise legally authorized provider.

The HBPC RN Case Manager makes scheduled home visits to the Veteran's residence. Self-motivation is the key to the pursuit of meeting professional work standards. The incumbent is required to maintain a valid driver's license. 50% to 75% of the job requires driving within a 50 mile radius of assigned duty station. The incumbent is responsible for providing remote support/supervision to the LVN for patient related issues. He/she collaborates with Nurses, Physicians, Social Workers, Dietitians, Program Support Assistants and other staff within the medical center to facilitate communication, continuity of care, inpatient-outpatient transition, and patient care flow management. This RN Interfaces with patients/family/significant others and members of the interdisciplinary team, consistently demonstrating skilled communication techniques, Assist to develop the initial nursing plan and updates the plan, serving as a resource person to less experienced staff, and acts as a leader in developing standards of care. He/she Leads or participates in care performance improvement activities that may include data collection, data analyses, reporting outcomes, evaluating outcomes, and implementing changes in practice based on qualitative and quantitative measurement criteria. Some of the performance measures include Customer Service Standards, Joint Commission, External Peer Review Program, and Clinical Guidelines Compliance and other standards of Quality of Care developed by the Care Line, medical center, and/or VHA. He/she Promotes, conducts or participates in evidence-based practice investigation designed to validate and/or improve patient care and health care delivery.

Work Schedule: Monday -Friday, 7:30am to 4:00pm; Alternate hours may be available.

BASIC REQUIREMENTS:

*U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

*Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

o The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)

* Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

*Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

*Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.

* Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

*Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

*Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.

Preferred Experience/Certification:Gerontology, CRRN (Certified Rehabilitation Registered Nurse), or Palliative Care Certificationis preferred.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Physical Requirements: Registered Nurse Functional Requirements: Heavy Lifting 50 lbs and over (intermittently); Carrying up to 45 lbs (intermittently); Repeated bending; Straight Pulling up to 45 lbs (intermittently); Straight Pushing up to 45 lbs (intermittently); Stooping (intermittently); Squatting (intermittently); Kneeling (occasionally); Use of both arms; Use of both hands; Use of fingers; Overhead work, both arms (intermittently); Prolonged walking (up to 4 hours/day); Prolonged standing (up to 4 hours/day); Sitting (up to 2 hours/day); Far Vision - correctable in one eye to 20/20 and 20/40 in the other; Near Vision - correctable to read newsprint at 13-16 inches; Ability to distinguish shades of colors; Hearing - ability to hear and understand the spoken voice from a distance of 8 feet. Ability to effectively utilize a stethoscope and distinguish breath and heart, and bowel sounds; Emotional and mental stability ?,- ability to effectively perform the essential functions of the position and respond appropriately in emergency situations.
