Licensed Practical Nurse - Community Living Center

The Sioux Falls VA Health Care System is currently recruiting forfour Full-time, Permanent, Licensed Practical Nurses. These positions will be located in the Extended Care & Rehabilitation Service. As a Licensed Practical Nurse,you are responsible for the following duties to include, but not limited to:

  • Demonstrate knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.
  • Recognize the need for and institute emergency measures when indicated, promptly seek assistance of the RN or provider, and assist in resuscitation procedures in cardiac and /or pulmonary arrest.
  • Ability to observe, identify and respond to the patient's needs for nursing interventions and recognizes the need to consider emotional, spiritual, socio-economic and age related factors.
  • In collaboration with the RN, participate in the assessment, planning, implementation, and evaluation of the nursing care that expedites the patient's return to, or maintenance of, optimal functioning.
  • Possess the ability to evaluate patient's status and interpret the appropriate information to identify each patient's age-specific needs and to provide the identified care.
  • Report and record observations and information related to the care of the patient and integrate the use of health care technology into the provision of nursing care.
  • Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspect of care delivery.
  • Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other customers.
  • Actively seeks out education opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.
  • Assist with range of motion, ambulating, lifting, positioning, and self-help activities.
  • Observe, report, and record obvious and less obvious changes in the patient's condition.
  • Clean and maintain patient's bedside units and ward work areas.
  • Accompany patients off station as requested, assisting with recreational and socialization activities.
  • The duties are performed in a hospital/extended care environment.

NOTE: Assignment to other areas of the service line, medical center, or other CBOC's may be required. The LPN reports to the CLC Nurse Manager and works in collaboration with his/her assigned team, however, assignments may be made to other areas for purposes of cross coverage.

Work Schedule: The tour of duty for this position istypically NIGHTS, rotating weekends and holidays are required. Flexibility in tours and days of the week are required. Additional information regarding the tour of duty for this position will be discussed during the interview process.

Basic Requirements:

  • Citizen of the United States.
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant.
  • Applicant must possess and maintain a full, active, current, and unrestricted license as a licensed practical or vocational nurse in a State, Territory or Commonwealth of the United States or in the District of Columbia.
  • Must be proficient in spoken and written English.

Grade Requirements:

GS-3:

- None beyond the Basic Requirements.

GS-4:

Education/Experience Required:

- Six months of qualifying experience as an LPN or LVN; OR

- Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; OR

- Graduation from an approved school of at least 24 months duration.

GS-5:

Experience Required:

- Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.

GS-6:

Experience Required:

- Completion of at least 1 year of qualifying experience at the GS-5 level or equivalent.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Grade Determinations:

Note: The full performance level of this vacancy is 06. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-03 to GS-06. The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.

PHYSICAL REQUIREMENTS: The work requires some standing, walking, bending, lifting, and carrying of light items. The work requires agility and dexterity and may require heavy lifting of equipment or patients. In many situations, the duration of the activity (such as most of a workday) contributes to the arduous nature of the job. Duties require extensive periods of standing and walking. Working with patients requires regular and recurring bending, lifting, stooping, stretching, and similar activities.
