Job Description
Social Worker - Clinical Coordinator

This position is within theDomiciliary Residential Rehabilitation Treatment Program (DRRTP)at the Bath VA Medical Center, Bath, NY. The Clinical Coordinatorprovides daily coordination of patient care activities within the domiciliary. Duties include, but are not limited to, the following:

Clinical Responsibilities:

  • Independently provides specialized psychological services at all levels of complexity for patients with PTSD, with or without comorbid substance use disorder(s), with service delivery to include intake evaluation, diagnostic assessments, psychotherapy and patient education.
  • Provides a full range of therapeutic interventions including individual therapy, group therapy, and psycho-education classes. Best practices, practice guidelines and evidence-based treatment protocols are utilized to the extent feasible in all therapeutic interventions.
  • Consults with medical center staff on a wide variety of patient care issues to include patient's decision-making capacity regarding health care.
  • Develops person-centered, culturally sensitive treatment plans to address mental health needs of patients.
  • Conducts evaluations to aid in the differential diagnosis of complex cases.
  • Complies with national and local performance measures, including timely health care screening and completion of clinical reminders for all patients.
  • Completes all clinical charting and documentation required by the Medical Center or healthcare system, as appropriate.
  • Responds to patients request for release of records; return telephone calls, linking of services, and appropriate referrals as needed in a timely fashion.
  • With permission from patient, collateral consultation with significant others/ support systems to aid in the facilitation of patients' treatment.

Administrative responsibilities:

  • Attends and participates in administrative and informational meetings held by the DRRTP Manager, Behavioral VA Health Care Line, Chief of Staff and Director.
  • Completes assigned Performance Improvement activities at the program, service line, and medical center level.
  • Attends regularly scheduled treatment team meetings to ensure efficiency and continuity of service delivery to specialized patient population.
  • Ensures program in compliance with regulatory and accreditation requirements and standards
  • Works collaboratively with the DRRTP manager to develop programing, policy and procedure for the DRRTP.


Work Schedule: Monday - Friday, 7:30 am to 4:00 pm.The incumbent may be contacted after hours by staff for guidance orevaluation of patient care issues.

Basic Requirements:

  • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Amaster's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Licensure or certification by a state to independently practice social work at the advanced practice level (LCSW or equivalent).
  • At least two years of experience in an area of specialized social work practice.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
