Job Description
Social Worker - Caregiver Support and Home Based Primary Care

This position is located at the Bath VA Medical Center in Bath, NY. The Social Worker provides support for the Caregiver Support Program and the Home Based Primary Care (HBPC) Program. Duties include the following:

  • Assist the HBPC Program and Caregiver Support Coordinator (CSC) with clinical programming and daily operations.
  • Provide clinical, evidence-based services and interventions, including home visits.
  • Educate caregivers, Veterans, and staff on caregiver issues.
  • Conduct outreach activities and presentations to the community.
  • Develop partnerships with local, state, and federal agencies, and with non-governmental agencies.
  • Perform resource development and quality improvement activities.
  • Support the CSC with the development of policies and procedures.
  • Prepare reports and statistics for facility and VA-wide use.
  • Operate a government vehicle to transport patients and travel to patients' homes.
  • Perform other related duties as assigned.


Work Schedule: Full-time. Monday - Friday, 7:30 AM - 4:00 PM

Salary:$60,210- 78,270.Salary to be determined by a Professional Standards Board (peer-to-peer review)based on applicant's education and qualifications.


BASIC REQUIREMENTS:

  • Must be a United States Citizen
  • Must be proficient in written and spoken English
  • Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Must be licensed or certified by a state to independently practice social work at the master's degree level.
  • A minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience). A doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting.


GRADE DETERMINATIONS:Appointment to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of clinical or health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level.

OR

In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting.


Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
