Job Description
Physician - Primary Care

We are seeking an immediate fill for full time Internal Medicine Physician / Family Medicine Physician, Board Certified, and preferably with experience in Gerontology to serve as a Home Based Primary Care Team Physician in the Home based Primary Care Program located at Sevierville, TN. This job involves providing face to face home based PC to veterans living within the catchment areas of Sevier county, Knox county, Campbell county and the other neighboring catchment areas.

The Physician will report directly to the Medical Director, HBPC.

The HBPC team physician in collaboration with the HBPC team is responsible for:

a. Assuming primary medical responsibility for their assigned panel of HBPC patients.

b. Identifying the patients' medical problems.

c. Defining the medical management of these problems.

d. Determining the need for consultation from medical, surgical, and/or psychiatric subspecialty clinics.

e. Determining the need for, and facilitating, admission to the hospital.

f. Performing home visits to deliver home based primary care to patients admitted to the HBPC Program.

g. Serving as collaborator for advanced practice nurses and physician assistants.

h. Providing medical administrative support such as signing forms requiring physician signature for the HBPC patients.

I. Participating in weekly interdisciplinary HBPC Team meetings.

j. Participating in Medical student/ Resident teaching.

J. Serving as clinical surrogate for HBPC Medical director

K. Participates in other clinical duties as assigned by the Medical Director HBPC and ACOS for Extended Care.

Work Schedule: 7:45 a.m. - 4:30 p.m. (Monday - Friday)

Permanent Change of Station (PCS)Expenses are not authorized for this position.

Basic Requirements:
  • Citizenship of the United States. Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with Department of Veterans Affairs policy.
  • Education. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Completion of a first-year residency, or its equivalent approved by the Department of Veterans Affairs for the year in which it was completed.
  • Licensure. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Residency Training. Physicians must have completed residency training or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA Physicians involved in academic training programs may be required to be board certified for faculty status). Approved residencies are: (1) Those approved by the accrediting bodies for graduate medical education, the Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA), in the list published for the year the residency was completed, or (2) Other residencies or their equivalents which the local Professional Standards Board determines to have provided an applicant with appropriate professional training.
  • English Language Proficiency. Must be proficientin spoken and written English.

Grade Determinations: The individual must qualify to perform duties as a physician, have completed an accredited residency and/or possess experience which has qualified the individual to perform general duties and some specialized functions and procedures without supervision. Staff physicians may also include attendings who train physician residents assigned in facilities with residency training programs, and consultants who are capable of giving authoritative views and opinions on subjects in their field of medicine.

Physical Requirements: Physicians should be physically and mentally capable to perform the examinations, formulate the diagnosis and treatment plan, and educate residents. Physical activity includes sitting, standing (up to 2 hours), walking (up to 2 hours), heavy lifting (45 pounds or over), reaching above the shoulder, and repeating bending. They should be emotionally and mentally stable and capable of performing the duties of the position.
